The Admissions Process
One of our Admissions Coordinators will meet with you or your caregiver during the admissions process to gather important information. During this time you or your guardian/POA will be required to fill a short admissions agreement. Office hours are 8:00-4:30 Monday through Friday. It is important that you provide us with the following documentation prior or during your admission to Kingsbrook:
- Medicare Card
- Medicaid Card
- Any other healthcare insurance card
- Social Security Card
- Advanced Directives, Living Will, Power of Attorney ( If Applicable )
- Power of Attorney Documents ( Clinical & Financial)
- Long-Term care/supplemental insurance policies
One of our admissions coordinators will review necessary state and federal required documentation as well as our admissions agreement and provide you with a copy.
